What does it mean to matter? It’s a deceptively simple question. Yet it reaches into virtually every part of our lives: our relationships, our work, our sense of purpose, and, importantly, our physical and emotional health. On this episode of The Empowering Neurologist, I speak with John R. Miles, host of the popular, Passion Struck podcast and author of the new book The Mattering Effect: Creating a Life of Meaning and Worth.

John’s exploration of mattering began with a deeply personal experience. At five years old, he sustained a traumatic brain injury that disrupted his speech, vision, coordination, and ability to learn. But as he explains, the most enduring injury was not simply neurological. It was the feeling that he had become someone to be managed and cared for rather than someone who had something meaningful to contribute.

John ultimately built an extraordinarily successful life, attending the United States Naval Academy, serving in the military, and becoming a corporate leader. Yet beneath those visible accomplishments, he began to disappear from his own life. His usefulness was continually reinforced, while his humanity was too often overlooked.

The moment that awakened him came when his nine-year-old son asked, “Dad, why don’t you ever laugh?” There had been no dramatic collapse. Rather, John recognized that he had slowly exchanged presence, joy, and authenticity for performance and outward accomplishment.

This distinction, between being useful and truly mattering, is central to our conversation today. Mattering is more than self-esteem, popularity, or receiving praise, what many us constantly pursue. It is the felt sense that our lives have value, that we are seen by others, and that, importantly, our presence and contributions make a difference. When this sense erodes, John argues, we begin paying what he calls the “Mattering Tax”: the exhausting cost of continually trying to prove that we deserve a place in rooms we already occupy.

This idea carries important implications for brain health. Human beings are profoundly social creatures. Loneliness, isolation, chronic stress, and the feeling of invisibility are not merely uncomfortable emotional states. They influence the nervous system, sleep, immune function, inflammation, cardiovascular health, and the way the brain perceives threat.

A life spent continually seeking validation may keep us productive while quietly keeping our stress response activated. John offers a practical alternative through his M.A.T.T.E.R. framework: Meaning, Autonomy, Trust, Time, Energy, and Reciprocity. Together, these six conditions help us develop relationships in which it is safe to be seen, protect our time and energy, and create a healthier balance between giving and receiving.

We also discuss how modern institutions can contribute to “unmattering.” Workplaces may reduce people to productivity metrics. In my world, I’ve seen healthcare turn patients into their named diagnoses. Technology can create the appearance of connection while depriving us of genuine presence. Even families can unintentionally communicate that worth depends on achievement, obedience, or usefulness.

But this is not simply a conversation about what has gone wrong. It is about what each of us can do.

We can listen without distraction. We can recognize another person’s contribution. We can make room for voices that have become quiet. And we can stop treating our own worth as something that must be earned through endless performance, a lesson I am only recently embracing.

John’s message is both timely and hopeful: Our worth is not a prize waiting at the end of achievement. It is already present. When we begin living from that understanding, and help others experience it as well, we change our relationships, our communities, and without question, our health.

This is a message well worth hearing. I hope you will join me for this deeply meaningful conversation with John R. Miles on The Empowering Neurologist.
